Supplemental Life
Insurance - CIGNA
Supplemental Life Insurance coverage is available to you at a
reasonable cost through Cigna Life Insurance Company. You may
purchase or increase/decrease your life insurance coverage
during Open Enrollment. If you elect new or additional life
insurance during Open Enrollment, please complete the Voluntary
Life Insurance Application below. If you purchase over $100,000
in coverage, you are required to also complete the Evidence of
Insurability (EOI) (medical
questionnaire) below. Please note that Unit III members may
only purchase up to $100,000 in Supplemental Life coverage.
The following are the forms necessary to enroll and/or make a
change in Supplemental Life Insurance benefits:
During Open Enrollment, the above
forms must be submitted to HR/Benefits by the end of the
Open Enrollment period.
