When you enroll your child in Park Elementary School, you will need to bring the following:

1. Your child’s birth certificate.

2. Your child’s immunization record (This must list all immunizations with the dates of each vaccination; be signed by a physician, health department official or school official; and be approved by the admitting school.)

* All doses must be given on or after the first birthday.

**One dose for children 24-59 months of age.

*** One dose of varicella (chickenpox) is required for a student younger than 13 years old. Two doses of varicella vaccine are required for a previously unvaccinated student 13 years or older.

****At least one dose given after 12 months of age.

All must be completed before students can attend school.

3. Proof of Residency: Either a lease, mortgage papers, or the

Tenant Verification form are mandatory along with a utility bill.

(You cannot use your driver's license.)

4. Please include any custody documentation regarding your child.

5. If you are enrolling your child for Pre-Kindergarten 2008-2009, your

child must be four years old on or before September 1, 2008.

6. If you are enrolling your child for Kindergarten 2008-2009, your child

be five years old on or before September 1, 2008.

7. Proof of income (1040 tax form) is necessary for Pre-Kindergarten

registration.
