Immunization records - proof of 2 MMRs 2. Proof(s) of residency (2)- Contract, lease, utility bill or bank statement 3. Proof of Custody 4. Birth Certificate 5. Social Security Card 6. Current or last report card 7. School Records - test scores, etc. 8. Special Education papers - IEP or 504 Crofton Middle School guidance department sponsors many programs for success, including Peer Mediation,  Student Ambassadors, Can-Do program, Advisory Lessons for the classroom teacher, Parent Involvement Center, and the Guidance Beacon, the monthly newsletter.  In addition, the counselors facilitate discussion groups for students centering around topics such as study skills, anger management, bully-proofing, coping with separation/divorce in the family, bereavement, living a healthy lifestyle, friendship, self-esteem, etc.     Outside programs available for students include: Maryland Hall for the Creative Arts Advanced Study Program at the Naval Academy Johns Hopkins Programs for Talented Youth Middle School Scholars Program
