Registration Requirements

Please call to schedule an appointment to register your child.

When you come to register, you will need to bring the following:

1. Immunization records - proof of 2 MMRs
2. Two proofs of residency - contract, lease, utility bill, or bank statement
3. Proof of Custody
4. Birth Certificate
5. Social Security Card
6. Current or last report card
7. School Records - test scores, etc.
8. Special Education papers - IEP or 504
