 KINDERGARTEN
REGISTRATION
At
Millersville Elementary School
Parents may register at the
office from 9:00 a.m. to 2:30 pm. Children who are five years of age on or
before September 1, 2008 are required to attend school. To register your
child, bring the following with you:
 | Child’s
original birth certificate |
| Copy
of the immunization record (a form will be given at this time to
be filled out by the Doctor) |
| TWO
proofs of residence ( one must be mortgage
bill, rental agreement or Lease agreement and the second can be phone
bill, electric bill, etc.) |
