• Apartment Lease, Mortgage Papers or Tenant Verification Form

Additional Documentation (at least one of the following is required):

• Utility Bill/Telephone bill, etc.
• Current bank statement
• Property tax/Income tax statement
• W-2 form
• Social Security check
• Domestic Relations check
• Passport
• Visa-entrance into the United States
• Voter’s registration

If applying to attend a school that is not your home school you will need to fill out an Application for Out of Area Transfer. Please see the attached form and requirements.
