PRE-KINDERGARTEN & KINDERGARTEN REGISTRATION

REGISTRATION BEGINS APRIL 14, 2008

TO ENROLL YOUR CHILD YOU MUST HAVE THE FOLLOWING:

1. ORIGINAL BIRTH CERTIFICATE

2. TWO PROOFS OF RESIDENCE:

MANDATORY - Apartment Lease, Mortgage Statement or Deed
OPTIONAL -	Utility Bill, Current Bank Statement, Property Tax, W-2 Form, Social Security Check, Current Pay Voucher, Domestic Relations Check

3. COMPLETED IMMUNIZATION RECORD

Kindergarten students registering for the 2008-2009 school year must be 5 years old by September 1, 2008.

Pre-kindergarten students registering for the 2008-2009 school year must be 4 years old by September 1, 2008.
